Student Case #1

We'll do two cases in one just for fun!

Pediatrics: 3 year-old girl presents with her mother with a 3 day history of fever, stuffy nose and cough. Fever has peaked at 103.4 F. What other history do you want to know? What kind of physical exam will you do?

Ob/GYN: 30 year-old G2P2 patient presents with irregular bleeding. What history do you want? What physical exam findings, if any, do you do?

Let's try to stick with history and physical findings for now, then we will move on the labs/diagnostics. I will only give you what you ask for!

can i play? i know only the bare bones of anat and phys but ill try.
im not sure what g2p2 is though =/

ob/gyn: date of lmp, is she on BC?(what kind if so), trying to conceive? painful intercourse? how long has she been bleeding? is spotting regular between periods?


exam: probably examine the vagina for any trauma, palpate the entire pelvic area.

i have no idea on the peds pt

oh id check BP also, temp for possible infection....draw blood (to test for pregnancy STD's) and urine. (make sure theres no filtrates)

G2P2 = gravida 2 para 2...in other words, pt has been pregnant twice, and has given birth twice. Often times in OB literature you will see GPFPAL with numbers behind them (gravida, para: full term, preterm, abortion, living).

Now the case...

LMP was 5 weeks ago. Periods have always been irregular. Currently not on OCP's, but not trying to conceive. Intercourse has never been painful, but she states she is not sexually active right now. Bleeding has been present for about 2 weeks. No spotting between periods. Pt states she has never experienced this before.

Before we get to the PE, any other history questions you'd like to ask?
